Abstract

Aim: Mediastinal neurogenic tumors (MNTs) are rare neoplasms derived from neural elements within the mediastinum. While most MNTs are benign and asymptomatic, surgical resection is generally recommended due to the potential for malignant transformation and complications related to tumor growth. This paper aims to provide a comprehensive overview of current surgical strategies for MNTs in adult patients, particularly focusing on benign lesions. Additionally, this review aims to establish a framework for surgical decision-making.
Material and Methods: This narrative review synthesizes data from recent literature on and clinical experience of the operative management of MNTs. It examines various surgical approaches, including minimally invasive and open techniques, with attention to technical details and case selection criteria.
Results: The review identifies key operative principles, highlights common challenges in the surgical management of MNTs, and evaluates the efficacy and safety of different techniques. It emphasizes the growing role of minimally invasive surgery in appropriately selected patients and discusses considerations for complex cases, such as tumors with intraspinal extension or hypervascularity.
Conclusion: A more structured and evidence-informed approach is required for the surgical management of MNTs. This review can guide thoracic surgeons by outlining technical strategies and procedural nuances that may improve outcomes and support individualized treatment plans for patients with these rare tumors.
