Abstract

Aim
To evaluate whether intravenous tranexamic acid (TXA) before unilateral biportal endoscopic spine surgery (UBE) can improve the visual field clarity, reduce postoperative pain and recovery quality.

Material and Methods
Analysis of the visual clarity, blood loss, and postoperative pain attenuation in patients undergoing UBE surgery with or without intravenous TXA administration.

Results
The study consisted of 98 patients (n = 56 with TXA, n = 42 without TXA). There were no obvious differences between the two groups for the patients baseline characteristics. The TXA group had shorter surgical duration and clearer visual field compared to the control group (P < 0.05). TXA significantly reduced intraoperative and postoperative blood loss (as indicated by changes in preoperative and postoperative hemoglobin and hematocrit levels) compared to the control group (P < 0.05). TXA significantly alleviated postoperative pain (as demonstrated by reducing immediate postoperative VAS scores and improving 1-week postoperative ODI scores compared to preoperative data) in TXA group (P < 0.05). However, there were no statistical differences of the immediate postoperative pain VAS scores, and the ODI score after operation at week 1 of the two groups (P > 0.05). There were no statistical differences between the two groups about the complications and occurrence (P > 0.05).

Conclusion
In UBE surgery, preoperative TXA enhances visual field clarity, shortens surgical time and blood loss, and encourages early functional recovery without increasing raising problems. These findings serve as valuable benchmarks for evaluating the efficacy of TXA aimed at enhancing patient visual field clarity in UBE spine surgery.
